Correct on 1 and 5. 2 is a decent deduction (Grum delivered like he was tossing Mills bombs) but quite a bit more recent.

Looks like he was using the thumb rather than the fingers to generate spin. It would also be a good grip to get it to go the other way.

In the recent 4th Test in Jo'burg, Faf de Plessis made a first ball duck and a ton in the same Test (0 & 120).
This has happened at least 2 dozen times in Tests, but only twice has a player made over 150 and managed a first ball duck in the other innings.
