I think rap is just the actual act of rapping. Fast, rythmic talking that rhymes.
Hip-hop is when you rap to a beat.
R & B is more complicated. It's changed over time and I can't be bothered to explain. Google it.

The boundaries are blurred really. You could say that the likes of 'Blackstreet' and the like are R&B, Hip-Hop essentially is rap, but I'd say Diddy and Kanye West are more HH because they sample loads of shit, someone like Wu Tang, with more basic beats and harder lyrics is rap.
