My chiropractor's surname is Nana. First time I've come across it. Anyone can tell me where in India that comes from?

you are spot on. I can speak for south Indians particularly Tamils. Father's first name comes first and the Son's name becomes second (surname). In the case of Murali Vijay , Murali = Father's first name . Vijay = Son's name. Another example is Mutthiah Muralitharan. Mutthiah is his father's name. Before moving to the UK I used this format but I changed it to Myname MyFather's name format. Caused a hell of a confusion in application forms etc. Only issue is that my children are going to inherit my father's first name as their surname.

Isn't nana a popular African name too? I have met fair few Africans particularly west africans with that name.
